APPLICATION INSTRUCTIONS

Below you will find the Application for the Y.E.S. Duluth program, which you can fill out onlineMake sure you fill in every field which is in a red box.  Fields in any other box or field is an optional field.  Make sure you read the instructions.  The Application is in Adobe .pdf format, so you will need to have the Adobe Acrobat Reader installed on your computer.  If you do not have the Reader, there is a link at the bottom of this page where you can download it and install it. 

There is a drop down sizing list on the upper right of the Adobe Control Panel where you can increase the page size of the form.  Be aware this may require your use of the scroll bars which may appear to scroll the form side-to-side or up-and-down. 

You will be required to digitally sign your application.  If you are under 18 years old, a parent will also be required to digitally sign your application.  Follow the instructions to create a digital signature.  A digital signature is just like signing a piece of paper only on your computer.

Again, you can fill out the form right on your computer, save it to your hard drive, then attach it to an email - the email address to send it to is either

Once we have reviewed your application we will call you to schedule an appointment for you to come in for an interview.  We will let you know at that time what to bring with you.

Also, there are 4 additional forms you will need to fill out and FAX or email to us  The FAX number is on each of the forms.  The email address is the same as the one in the above paragraph.

NOTE:  The following are the instructions to follow to Digitally Sign your application once you have finished filling it out.

  • When you click your mouse in the Red Box to sign your name, a “Sign Document Window” will appear.
  • Type your name into the “Sign As” box, or
  • Click on the V arrow on the Right side of the box, and click on “New ID.”
  • In the “New Id” window, click on the Radio Button next to “A New Digital ID . . .,” then click “Next.”
  • Click “Next” again, leaving the button already checked as is.
  • In the “Add Digital ID” window, make sure your name appears in the name field, and enter your email address in the “Email Address” field.
  • Click Next.
  • Enter and re-confirm a password you create (make sure you write it down; you will need it in the next window).
  • Click “Finish.”
  • In the “Sign Document” Window, enter the password you created above and click on “Sign” to Digitally sign your application.
  • You can now print out a copy and save the application to your computer.  If you have any questions about this process, call our office for help.

If you do NOT have the Adobe Reader on your computer, or are having trouble viewing the Application or the other forms in Adobe pdf on this web site, click on the Adobe Reader icon below to download it and install it on your computer
